The FPV HREC currently uses the Common Application Form http://www.health.vic.gov.au/ethics and is trialling the use of the online National Ethics Application Form (NEAF) http://www.neaf.gov.au Please refer to the National Statement 2007 when making applications.

Prior to completing these forms, a Preliminary Application to Conduct Research at FPV Form must be completed. Please contact the HREC Administrative Officer Judy Earley on
03 9257 0152 or email jearley@fpv.org.au for information regarding this.

The purpose of the Preliminary Application Form is to determine the organisational capacity and priority of FPV being involved in a specific research project.

You may use either the NEAF or Common Application Form in making your application for ethics approval for your research project.

Please note that applicants using the NEAF online application form will need to create a user name and click on "new proposal".

Both the Common Application Form and NEAF application users must complete the FPV Addendum to the Common Application
Form
for FPV specific requirements.
